29 CFR  1926.416(a)(1):  Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means.  (a) Location - 660 Bridge, Lowell, MA:  On or about 7/15/2021,  employees were exposed to electric shock and/or electrocution hazards while performing roof replacement activities in the direct vicinity of energized electrical circuits.

(a) Location - 660 Bridge Street, Lowell, MA:  On or about 7/15/2021, employees were allowed to use body belts as a primary means of personal fall arrest, thereby exposing employees to internal injuries from falls greater than 20 feet.

29 CFR  1926.502(d)(15):  Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.  (a) Location - 660 Bridge, Lowell, MA:  On or about 7/15/2021, employees were not protected from falling when more than one employee was attached to a single anchor.

29 CFR  1926.502(d)(17):  The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.  (a) Location - 660 Bridge, Lowell, MA:  On or about 7/15/2021,  employees were exposed to falls grater than 20 feet while utilizing life lines connected directly to the front/waist positioning ring and not using D-rings located at the center of back.
