29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits that were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means.  Location: 208 Highland Ave, Malden, MA  On or about 02/21/2023, employees were permitted to perform siding activities from an Alum-A-Pole pumpjack scaffolding system that was in proximity (3-feet) to exposed energized power lines.  Evolution Roofing Corp was previously cited for a violation of this occupational safety and health standard, 1926.416(a)(1), which was contained in OSHA inspection number 1542051, citation number 1, item number 1 and was affirmed as a final order on 12/01/2021, with respect to a workplace located at 660 Bridge Street, Lowell, MA.

29 CFR 1926.452(j)(2): Poles of pump jack scaffolding were not secured to the structure by rigid triangular bracing, or equivalent, at the bottom as necessary.  Location: 208 Highland Ave, Malden, MA;  On or about 02/21/2023, the employer erected an Alum-A-Pole pumpjack scaffold and did not secure the bottoms of the four pole sections with rigid triangular bracing, or equivalent means and exposed employees to a potential fall or struck-by hazard if the poles kicked out or slid out of place on the concrete walkway which they were erected on top of.

Location: 208 Highland Ave, Malden, MA  On or about 02/21/2023, the employer allowed an Alum-A-Pole pumpjack scaffolding system to be erected on-site by employees who were not trained or experienced and had no competent person who was qualified in scaffold erection, moving, dismantling or alteration on-site.  Evolution Roofing Corp was previously cited for a violation of this occupational safety and health standard, 1926.451(f)(7), which was contained in OSHA inspection number 1464173, citation number 1, item number 1 and was affirmed as a final order on 05/11/2020, with respect to a workplace located at 1190 Bedford St, Whitman, MA.
