29 CFR 1926.106(a):Employees working over or near water where the danger of drowning existed, were not provided with U.S. Coast Guard approved life jackets or buoyant work vests: (a) On or about July 13th, 2021, at the bridge pile of the Browns Bridge, employees were exposed to drowning hazards from falling into the Racket River while working to remove debris at the edge of the pile. Employees were not provided with life jackets or vests.

29 CFR 1926.106(c):Ring buoys with at least 90 feet of line were not provided and readily available for emergency rescue operations: (a) On or about July 13th, 2021, at the bridge pile of the Browns Bridge, employees were exposed to drowning hazards from falling into the Racket River while working to remove debris at the edge of the pile. The ring buoy located on site did not have a line attached to it to facilitate rescue operations.
