29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to the outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): On or about July 16, 2021, extension cords connected to QC liner press, and tape/roller machine #1 were frayed and showing inner insulation with several damaged areas throughout the length of both cords. The condition of the extension cords demonstrates a lack of a visual inspection before use.

On or about June 22, 2021, employees were using visibly damaged extension cords with the QC liner press, and tape/roller machine #1. Cords were heat damaged or the outer insulation was damaged/pulled away from the strain relief exposing inner wiring.
