29 CFR 1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about July 15, 2021, at a commercial construction site in Cypress, TX, employees were exposed to struck-by hazards while cutting framing lumber with a powered saw without eye or face protection.

29 CFR 1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about July 15, 2021, on the roof of a commercial building being constructed in Cypress, TX, employees were exposed to a fall hazard of more than 6 feet without a fall protection system in place.
