29 CFR 1926.600(a)(6)(i): For lines rated 50kV or below, a minimum of 10 feet was not maintained between the energized line and the equipment: This violation was most recently observed on or about July 12, 2021, at a worksite located at 117 Morgan Ave., Aurora, Missouri, where an employee exposed himself and other employees at the worksite to shock and electrocution hazards. The pumper truck�s extendable arm came within 10 feet of energized overhead power lines causing the equipment to become energized and subsequently shocked and burned other employees at the worksite in contact with the equipment.

29 CFR 1926.701(b):All protruding reinforcing steel, onto and/or into which employees could fall, was not guarded to eliminate the hazard of impalement: Worksite located at 117 Morgan Ave., Aurora, Missouri- On or about July 12, 2021, an employee was working and walking around the block foundation which had protruding reinforcing steel sticking out in which the steel was not guarded to protect from impalement hazards.
