29 CFR 1926.502(a)(2): The employer did not provide and install all fall protection systems required by this subpart for an employee, and did not comply with all other pertinent requirements of this subpart before that employee began the work that necessitates the fall protection: a) Dominion Terminal Associates: On or about 07/12/2021, an employee was tasked with welding the backside of a deflector plate without enough attachment points capable of supporting at least 5000 pounds, as a result disconnected his lanyard and fell approximately 20 feet. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

A 5/8 inch stainless steel cable was looped around a metal frame, extended above a walk board, looped through a pad-eye and fastened using three (3) U-bolt saddle clamps. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
