29 CFR 1910.151(c):Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a) Battery Room # F503: The path to the emergency eyewash/shower was blocked by batteries stored at the wall and forklifts which were parked near them. The openings to allow passage were only between 12 inches and 22 inches also battery cables ran from the wall to the parked forklifts which blocked any employee from gaining access to the eyewash/shower during an emergency.

29 CFR 1910.132(a):Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered. a) Worksite: New replacement seats added to some forklifts used at the site were found to be of an increased height which caused some employees to come into contact with (or a few inches below) the overhead guard. The employer did not provide a means to prevent employees from bumping the top of their heads.
