On or about 07/16/2021, at the job site - employees applying stucco from the top level of the scaffold were exposed to a 14 foot fall hazards, in that, the scaffold was missing the end cap guardrails.

29 CFR  1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.   a. On or about 07/16/2021, at the job site - employees conducting stucco work from a scaffold were exposed to a 14 foot fall hazard, in that, they were not provided with training.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a. On or about 07/16/2021, at the job site - an employee conducting stucco work from a wood board placed with one end on the second level of the scaffold and the other end on top of the roof was exposed to a 9 foot 11 inch fall hazard to the outside ground and a 9 foot 3 inch fall hazard to the inside concrete floor while working from the top of the block walls without fall protection.
