Luis Castillo, on or about July 19, 2021 at the address of 5635 Waterville Monclova Rd. Monclova, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as preventing falls from heights on the worksite. Luis Castillo was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.20(b)(1), which was contained in OSHA inspection number 1416324, citation number 1, item number 1a and was affirmed as a final order on August 6, 2019, with respect to a workplace located at 503 E. Madison St. Sandusky, OH.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Luis Castillo, on or about July 19, 2021 at the address of 5635 Waterville Monclova Rd. Monclova, OH: the employer did not ensure employees were protected from falls while performing roofing activities for a residential type structure. Employees were exposed to up to 12' falls to the ground below in that their personal fall arrest systems did not comply with 1926.500(b). Luis Castillo was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1416324, citation number 1 item number 2 and was affirmed as a final order on August 6, 2019, with respect to a workplace located at 503 E. Madison St. Sandusky, OH.
