29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: (a) Tower B roof; An extension cord did not have strain relief which led to exposed wires at the socket end. A Makita reciprocating saw was plugged into the socket and used to cut out drain holes in the roof.
