On or about July 13, 2021 at the job site located at 175 Hickory Lane, Moreland Hills, Ohio 44022; The 8 foot self-supporting ladder was set up to gain access to the elevated area where the employees were drilling 2, 3 ? inch holes through the exterior of the new construction residential structure. The self-supporting ladder was set up and accessed on uneven ground which caused it to displace.

On or about July 13, 2021 at the job site located at 175 Hickory Lane, Moreland Hills, Ohio 44022; employees were engaged in work activity that included drilling 2, 3 ? inch holes through the exterior of a residential structure while gaining access to the elevated working height by utilizing the top 2 steps on a self-supporting stepladder.

On or about July 13, 2021 at the job site located at 175 Hickory Lane, Moreland Hills, Ohio 44022; employees were engaged in work activity that included drilling 2, 3 ? inch holes through the exterior of a residential structure while gaining access to the elevated working height by utilizing the cross-bracing on the rear side of the self-supporting ladder as the elevated employee straddled the ladder to gain access.
