29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided. On July 19, 2021, the employer did not provide an extension ladder where employees could access and egress the residential building second level, exposing them to potential fall hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On July 19, 2021, employees engaged in framing work without fall protection were expose to unprotected fall hazards of approximately 17 feet to the ground below. Alberto Torres Vazquez was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13), which was contained in OSHA inspection number 1465130, citation number 1, item number 2 and was affirmed as a final order on June 15, 2020, with respect to a workplace located at 148 Caurel Cir, Little Rock, AR 72223.
