29 CFR  1926.100(a):Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  SNS Absolute Roofing, LLC DBA SNS Absolute Construction at the worksite located at 4265 Keygate Dr., in Toledo, Ohio: On or about July 19, 2021, the employer did not ensure employees were protected from head injuries while working below employees performing roofing operations.  Employees tasked with clean-up operations were exposed to potential impact injuries from falling material.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR  1926.20(b)(1):The employer did not initiate and maintain such programs as may be necessary to comply with this part.  SNS Absolute Roofing, LLC DBA SNS Absolute Construction at the worksite located at 4265 Keygate Dr., in Toledo, Ohio: On or about July 19, 2021, the employer did not have a plan or program to identify job site hazards for employees to maintain a safe working environment.  Such a program would allow the employer to identify falls and falling object hazards during job site operations, decreasing the probability of employee injury.  SNS Absolute Roofing, LLC DBA SNS Absolute Construction was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.20(b)(1) which was contained in OSHA inspection number 1227351, citation number 2, item number 1 and was affirmed as a final order on 8/7/2017, with respect to a workplace located at 4150 Laskey Rd., Toledo, Ohio 43623.   SNS Absolute Roofing, LLC DBA SNS Absolute Construction was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.20(b)(1), which was contained in OSHA inspection number 1194279, citation number 1, item number 1a and was affirmed as a final order on 3/29/2017, with respect to a workplace located at 1740 E. Wooster St., Bowling Green, Ohio 43402.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

SNS Absolute Roofing, LLC DBA SNS Absolute Construction was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1227351, citation number 2, item number 2 and was affirmed as a final order on 8/7/2017, with respect to a workplace located at 4150 Laskey Rd., Toledo, Ohio 43623.  SNS Absolute Roofing, LLC DBA SNS Absolute Construction  was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1194279, citation number 1, item number 1b and was affirmed as a final order on 3/29/2017, with respect to a workplace located at 1740 E. Wooster St. Bowling Green, Ohio 43402.
