The crew lead and three employees were not utilizing eye protection while using pneumatic nail guns to fasten shingles to the roof.

On or about 7/19/2021, employees from Roman Guzman were installing shingles on a residence located on lot 376 on Victory Height Drive in Wentzville, MO. They were not utilizing guardrail systems, a safety net system, or personal fall arrest systems. The crew lead and three employees were observed working from a 7/12 to 10/12 sloped roof approximately ten to seventeen feet above the ground without utilizing fall protection.

On or about 7/19/2021, employees from Roman Guzman were installing shingles on a residence located on lot 376 on Victory Height Drive in Wentzville, MO. The crew lead and three employees were using an extension ladder to access the roof. The ladder was not extended three feet beyond the landing surface or edge of the roof.
