29 CFR  1910.178(a)(4):Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly:   a) On or about July 20, 2021, at an establishment located at 504 Willow Drive in Spencer, Wisconsin; the employer did not obtain written approval from the manufacturer prior to the installation of eight foot forks on the JLG G9-43A telehandler. The capacity chart supplied on the vehicle did not include lifting capacity limitations nor safe operating instructions for the eight foot forks installed.  b) On or about July 20, 2021 the employer did not obtain written approval from the manufacturer prior to the modification of the carriage assembly. The slotted brace had been removed from the assembly that ensured the fork pin would not become dislodged during use, resulting in potential pin failure.

29 CFR  1910.178(f)(2):The storage and handling of liquefied petroleum gas fuel shall be in accordance with NFPA Storage and Handling of Liquefied Petroleum Gases (NFPA No. 58-1969), which is incorporated by reference as specified in Sec. 1910.6:   On or about July 20, 2021, at an establishment located at 504 Willow Drive in Spencer, Wisconsin; the liquefied petroleum (LP) cylinder mounting bracket was not securely attached to the Komatsu 30 FG30G211 forklift to ensure that the LP cylinder did not jar loose, slip and/or rotate while in use, exposing the operator to potential struck by and fire hazards.
