On or about July 27, 2021, on the east side fabrication shop area, an employee was using a Milwaukee 4 1/2 inch angle grinder without an adequate guard while performing fabrication tasks.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to the outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). On or about July 27, 2021, an extension cord that was in use had the outer insulation damaged exposing inner wiring.

29 CFR 1910.334(a)(3)(i): A flexible cord used with grounding type equipment did not contain an equipment grounding conductor: On or about July 27, 2021, three equipment power cords used to power fans were without ground prongs. Instance A) Power cord used to power the thirty-inch (30) orange shop fan. The fan is the orange shop fan. Instance B) Power cord used to power the 12 in. all Metal Floor Fan fan perched on the pedestal. The fan has no identifying marks. Instance C) Power cord used to power 14 in. 3-Speed High Velocity All Metal Tilting Floor Fan.
