The Husky portable light equipment was plugged into an Stanly power strip which was plugged (daisy chained) into an extension cord. Employees were exposed to potential electrical shock and /or fire hazard(s).

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: Casino Bar: On or before July 20, 2021 employees used an Husky 2000-Lumen Portable LED Work Light for lighting during a bar renovation. The Husky work light plug was pulled away from the insulation exposing the inner insulated wires. Employees were exposed to electrical shock hazard(s).
