29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns. On or about July 21, 2021, and at times prior thereto, workers performing roofing activities on two separate buildings were not protected with hard hats after descending the roof to the ground, exposing the roofers to the hazard of being struck-by falling tools, materials and debris.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On or about July 21, 2021, and at times prior thereto, roofers were using pneumatic nail guns to perform roofing activities without the use of safety glasses, exposing the roofers to the hazard of being struck-by flying projectiles and particles to the eye(s).

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about July 21, 2021, and at times prior thereto, an aluminum extension ladder was used to gain access to the upper landing of the Protho Center without being extended at least 3-feet above, exposing the employees accessing the roof to a fall hazard.
