On or about July 21, 2021, and at times prior thereto, employees performing roofing activities on two separate buildings were not protected with hard hats after descending the roof to the ground, exposing the roofers to the hazard of being struck-by falling tools, materials and debris.

On or about July 21, 2021, and at times prior thereto, five roofers were performing roofing activities without using fall protection, exposing the roofers to a fall hazard of over 30-feet to the ground below. MCM Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.501(b)(10)), which was contained in OSHA inspection number 1401464, citation number 01, item number 01 and was affirmed as a final order on 07/25/2019, with respect to a workplace located at 2712 Satsuma Drive, Dallas, TX 75229.

On or about July 21, 2021, and at times prior thereto, an aluminum extension ladder was used to gain access to the upper landing of the Protho Center without being extended at least 3-feet above, exposing the employees accessing the roof to a fall hazard.
