The employer did not ensure that handrails and the top rails of stair rail systems are capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied in any downward or outward direction within 2 inches (5 cm) of any point along the top edge of the rail. a) Stairs to Room 114 - Stairs to Room 114 - On or about July 19, 2021, the employees used stairs to access the upper and lower landings, but the handrail was not securely attached near the bottom step, exposing employees to fall hazards.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a) Hallway - On or about July 19, 2021, employees were provided an emergency eyewash station manufactured by Watersaver Faucet Company to flush eyes in the event of an emergency. The eyewash station was not suitable in that it was not inspected and maintained in accordance with manufacturer requirements, exposing employees to biological hazards. b) Chemical Lab 201 - On or about July 19, 2021, employees performed research activities with the use of corrosive chemicals to include but not limited to sodium hydroxide. The eyewash/drench hose station manufactured by Watersaver Faucet Company was provided for emergency use, but was not suitable in that it was not inspected and maintained in accordance with manufacturer requirements, exposing employees to biological hazards. c) Chemical Lab 203 - On or about July 19, 2021, employees performed research activities with the use of corrosive chemicals such as but not limited to methyl chloroformate and bleach. The eyewash/drench host station manufactured by Watersaver Faucet Company was provided for emergency use, but was not suitable in that it was not inspected and maintained in accordance with manufacturer requirements, exposing employees to biological hazards. d) Genetics Lab 212 - On or about July 19, 2021, employees were provided an emergency eyewash station manufactured by Watersaver Faucet Company to flush eyes in the event of an emergency. The eyewash station was not suitable in that it was not inspected and maintained in accordance with manufacturer requirements, exposing employees to biological hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Maintenance Shop - On or about July 19, 2021, employees operated a 18 inch variable speed drill press manufactured by Powermatic (model: 2800, serial number: 6100007). The point of operation was not guarded to protect the operator from the rotating chuck and swarf produced during operation, exposing employees to flying swarf and caught-in hazards. b) Maintenance Shop - On or about July 19, 2021, employees operated a 10 inch table saw manufactured by Bosch (model TS2000, Serial number: 69018 3867) for cutting operations. The table saw was not provided a guard to protect operators from inadvertent contact with the blade at the point of operation, exposing employees to caught-in hazards. The National Oceanic and Atmospheric Administration (NOAA) was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 1216287, citation number 1, item number 2 and was affirmed as a final order on April 17, 2017 with respect to a workplace located at 7851 Dean Martin Drive in Las Vegas, Nevada.
