29 CFR  1910.22(a)(1):29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition.  a) Machine Area - On or about July 20, 2021, items were observed stored in the walking-working concrete flooring to include but not limited to hoses and electrical cables, limiting egress and exposing employees to tripping hazards.

29 CFR  1910.151(b): First aid supplies approved by a consulting physician were not readily available:  a) First Aid Center/Outside of Office - On or about July 20, 2021, the employer provided employees with antiseptic first aid spray manufactured by Physicians Care which had expired on December 2017 and was no longer adequate for application.   b) First Aid Kit/Outside of Office - On or about July 20, 2021, employees were provided with Ophthalmic Solution Eyewash manufactured by Physician Care for cleaning and flushing of eyes in the event of an emergency, but the eyewash expired April 2018 and was no longer suitable for use.  c) First Aid Kit/Warehouse Garage - On or about July 21, 2021, employees were provided with eye saline solution manufactured by Honeywell for flushing or irrigating the eyes in the event of an emergency, but the eye saline solution expired August 2013 and was no longer suitable for use.

29 CFR  1910.151(c):Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.  a) Warehouse Garage - On or about July 21, 2021, the employer provided a plumbed emergency eyewash station for quick flushing of eyes in  the event of an emergency.  The eye covers had been removed and not replaced, allowing for dirt and other particulates to accumulate on the eyelets, exposing employees to biological hazards.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Machine Area - On or about July 20, 2021, employees operated a Kent USA drill press (model #: KTM5VKF, SN: 10212690), which was unguarded and not protected at the point of operation, exposing employees to  caught-in hazards and flying swarf.  b) Machine Area - On or about July 20, 2021, employees operated an American Turnmaster lathe labeled by the U.S. Coast Guard 2KM67 which had the guard removed at the chuck, exposing employees to caught-in hazards and flying swarf.

29 CFR  1910.303(g)(1)(i): Work space was less than 30 inches wide in front of electric equipment operating at 600 volts, nominal, or less:   a) Machine Area/Green Desk - On or about July 20, 2021, the H-12 three phase panel had items placed in the working space to include a green desk and chair, exposing employees to fire and electrical hazards.  b) Maintenance Area - On or about July 20, 2021, the I-4 three phase panel had items stored within the 30 inches to include lamp, paper, and cords, exposing employees to fire and electrical hazards.  c) Grinding Area - On or about July 20, 2021, a 208 Volt electrical panels (J-2) had combustible items and a compressed gas cylinder stored directly in the working space, exposing employees to fire and electrical hazards.  d) Machine Shop - On or about July 20, 2021, a 208 Volt electrical panel (A) had combustible items items stored directly in the working space, exposing employees to fire and electrical hazards.  e) Breakroom - On or about July 20, 2021, a 208 Volt,  panel (V) had combustible items stored directly in front of the working space, exposing employees to fire and electrical hazards.

29 CFR  1960.8(b): The Agency did not comply with the Occupational Safety and Health Administration standards applicable to the agency :  a) Maintenance Area - On or about July 20, 2021, employees performed vehicle maintenance in an area where a metal racking system containing tires, chemicals, tools, and other items were stored.  Interior and rear column base plates on the frames of the racking system were not anchored to the floor to resist all applicable forces, exposing employees to struck-by and crushing hazards.
