29 CFR  1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  29 S. Bedford St., Woburn, MA On or about, July 17, 2021, employees were exposed to being struck in the head from falling tools, equipment solar panel parts while working on the ground and from ladders below the workers installing the solar panel system on the roof.

29 CFR  1926.416(a)(1) :Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  29 S Bedford St., Woburn, MA  On or about, July 17, 2021, employees were exposed to an electrocution hazard  while installing a solar panel system on a residential roof near an energized service drop that was not insulated, covered or guarded by insulation.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   29 S Bedford St., Woburn, On or about, July 17, 2021, employees were exposed to falls of up to 22 feet while installing a solar energy system on a residential single-family house roof without fall protection.  The Sunrun Solar, Inc. company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1342329,  citation number 1, item number 1 and was affirmed as a final order on March 12, 2019, with respect to a workplace located at 63-19 90th Street, Howard Beach, NY 11414.

29 CFR  1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used:   29 S Bedford St., Woburn, On or about, July 17, 2021, employees were exposed to falls of up to 22 feet while installing a solar energy system on a residential single-family house roof were not adequately trained in the installation, used and limitations of the personal fall arrest and restraint equipment, anchorage installation and use, the allowable working zones to reduce the risk of swing falls and improper loading.  The Sunrun Solar, Inc. company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.503(a)(2)(ii), which was contained in OSHA inspection number 1191351,  citation number 1, item number 1B and was affirmed as a final order on January 23, 2017, with respect to a workplace located at 19 Pine Knoll Road, Southwick, MA 01077.
