29 CFR 1910.26(c): Portable dock boards are secured by anchoring them in place or using equipment or devices that prevent the dock board from moving out of a safe position. When the employer demonstrates that securing the dock board is not feasible, the employer must ensure there is sufficient contact between the dock board and the surface to prevent the dock board from moving out of a safe position; On or about the week of July 12, 2021, employees utilized the Uline 72" x 60" dock board to access the trailers for loading and unloading exposing employees to fall and tipping hazards.

Powered industrial truck operators shall receive initial training in the following topics, except in topics which the employer can demonstrate are not applicable to safe operation of the truck in the employer's workplace. On or about July 1, 2021 and thereafter, employees operating the Yale propane forklift (Lift Truck Model: GLC040ABJVAE077, S/N: 431873) were not trained and evaluated prior to operating to ensure they had the skills and knowledge to operate safely.

29 CFR 1910.178(o)(2): Only loads within the rated capacity of the truck shall be handled. On or about the week of July 12, 2021 and thereafter, the Yale propane forklift (Lift Truck Model: GLC040ABJVAE077, S/N: 431873) unloaded four skids of conveyor parts which weighed a total of approximately 19,000 pounds (4,750 pounds per skid) which had a maximum capacity of 3,800 pounds.

Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse. (a) Pallet racking located on the south wall by the main overhead door was not secured to prevent tipping. (b) Pallet racking located in the center bay was not secured to prevent tipping. (c) Pallet racking located on the south wall by the computer was not secured to prevent tipping.
