On or about July 21, 2021, the midrail was not installed midway between the top edge of the guardrail system and the walking-working surface.  Employees were exposed to falls of 7.5 feet to the concrete floor below while working from the upper deck to clean the plates of  the Novoflex Flexographic press RF01 with a toprail at 43" and a midrail at 27".  b.  On or about July 21, 2021, the midrail was not installed midway between the top edge of the guardrail system and the walking-working surface.
