On or about July 21, 2021, three employees were using a pneumatic nail guns to secure shingles to a roof without wearing appropriate eye protection, exposing the employees to the hazard of being struck in the eye(s) by flying debris or projectiles.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor an alternative fall protection measure that meets a provision described in paragraph (b) of this section. On or about July 21, 2021 and at times prior thereto, six employees were installing shingles without the use of fall protection up to approximately 25 feet from the ground, exposing the employees to a fall hazard.

On or about July 21, 2021, roofers accessed the roof of a two-story home by using a portable ladder that was not extended at least 3 feet above the upper landing surface, exposing the roofers to a fall hazard of approximately 10 feet to the ground below.
