29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee was using eye or face protection when exposed to eye or face hazards from flying particles.  On or about July 21, 2021, three employees were using a pneumatic nail guns to secure shingles to a roof without wearing appropriate eye protection, exposing the employees to the hazard of being struck in the eye(s) by flying debris or projectiles.

29 CFR  1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor an alternative fall protection measure that meets a provision described in paragraph (b) of this section.  On or about July 21, 2021 and at times prior thereto, six employees were installing shingles without the use of fall protection up to approximately 25 feet from the ground, exposing the employees to a fall hazard.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  On or about July 21, 2021 and at times prior thereto, six employees were installing shingles at heights over six feet without receiving training from the employer, exposing the employees to a fall hazard.

29 CFR  1926.1053(b)(1):29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  On or about July 21, 2021, roofers accessed the roof of a two-story home by using a portable ladder that was not extended at least 3 feet above the upper landing surface, exposing the roofers to a fall hazard of approximately 10 feet to the ground below.
