29 CFR 1926.102(a)(1):The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation. On or about July 21, 2021, two employees were using pneumatic nail guns to secure decking to a roof without wearing appropriate eye protection, exposing the employees to the hazard of being struck in the eye(s) by flying debris or projectiles.

29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. On or about July 21, 2021, and at times prior thereto, three roofers were installing shingles on the roof of an existing home up to approximately 12 feet high without fall protection, exposing the employees to a fall hazard.
