On or about 07/22/2021, at the milling area of the job site - the doors interlocks to the MORI SEIKI 5100 and the NL 2000 lathe were not functioning, which allowed the machines to continue operating while the doors were open, exposing employees to point of operation, ingoing nip points, rotating parts, and flying chips hazards.
