29 CFR  1910.95(c)(1):The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  a.   A thread inspector on line 1 was exposed to continuous noise at 132.3% of the daily noise exposure (8-hour time-weighted average sound level of 90 dBA) or an equivalent sound level of approximately 92 dBA during the 450 minute sampling period on July 22, 2021; exposure calculations included a zero increment for the 30 minutes not sampled.  b.   A thread inspector on line 2 was exposed to continuous noise at 152.7% of the daily noise exposure (8-hour time-weighted average sound level of 90 dBA) or an equivalent sound level of approximately 93 dBA during the 425 minute sampling period on July 22, 2021; exposure calculations included a zero increment for the 55 minutes not sampled.  c.   A thread inspector on line 3 was exposed to continuous noise at 97.3% of the daily noise exposure (8-hour time-weighted average sound level of 90 dBA) or an equivalent sound level of approximately 89.8 dBA during the 444 minute sampling period on July 22, 2021; exposure calculations included a zero increment for the 36 minutes not sampled.  d.   A thread inspector on line 4 was exposed to continuous noise at 217.6% of the daily noise exposure (8-hour time-weighted average sound level of 90 dBA) or an equivalent sound level of approximately 95.6 dBA during the 444 minute sampling period on July 22, 2021; exposure calculations included a zero increment for the 36 minutes not sampled.

29 CFR  1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  On or about July 6, 2021, on line 2, employees were exposed to caught-in and struck-by hazards when performing service and /or maintenance on the Mazak Slant Turn and the Superior Type 3 Bucking Unit without an established energy control program.
