On or about July 19, 2021, at the above addressed jobsite, an employee working on the ground, below tuckpointing operations, was not wearing a hard hat to protect against falling materials and debris.

On or about July 19, 2021, at the above addressed jobsite, employees performing tuckpointing operations, three (3) stories above the ground, were exposed to falls, due to the employer not installing guys, ties, and/or braces into the structure.

On or about July 19, 2021, at the above addressed jobsite, employees were performing tuckpointing operations from a tubular welded frame scaffold, three (3) stories above the ground, that was not provided with mud sills, thereby exposing employees to falls.
