29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) Jobsite 3401 Health Sciences Dr., Philadelphia, PA 19104 - On or about July 21, 2021, the employer did not instruct each employee in the recognition and avoidance of unsafe conditions at the jobsite, and the regulations to control or eliminate pinch point hazards, when receiving and moving elevator rail bundles. An employee suffered a finger amputation when his fingers got caught between the bundle of rails and the inside of the cart used to move the bundles.
