29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation.  a) On July 20, 2021, at the above addressed jobsite, employees were exposed to eye injury while operating air nailer guns without eye protection while installing a new roof on a residential home.  No abatement certification or documentation is required for this item.

29 CFR  1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall, was not guarded to eliminate the hazard of impalement:  a) On July 20, 2021, at the above addressed jobsite, employees were exposed to an impalement hazard from exposed rebar that protruded from poured concrete around the window wells.  No abatement certification or documentation is required for this item.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) On July 20, 2021, at the above addressed jobsite, employees conducting installation of a roof on a new construction residential home, at approximately eighteen (18') feet ten (10) inches above the ground, were not provided personal fall arrest systems or other means of fall protection, thereby exposing the employee to fall hazards.  No abatement certification or documentation is required for this item.    Dynamic Home Exteriors, Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1438869, citation number 1, item number 2, and was affirmed as a final order on November 19, 2019, with respect to a workplace located at 826 Woodland Heights Boulevard in Streamwood, Illinois.
