29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) On July 20, 2021, at the above addressed jobsite, employees were exposed to a struck-by hazard from falling joists, lumber, tools, and debris when working below employees and material.  The employees were not wearing hard hats.  No abatement certification or documentation is required for this item.

29 CFR  1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  a) On July 20, 2021, an employee was to the hazard of falling approximately twenty (20) feet five (5) inches from an aerial lift platform because a full body harness was not worn with a lanyard attached to the boom or basket while working from and climbing out of an aerial lift.   No abatement certification or documentation is required for this item.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) On July 20, 2021, at the above addressed jobsite, employees conducting installation of joists in a new construction residential garage, at approximately twenty (20) feet five (5) inches above the lower level of the garage and approximately ten (10) feet to the main level of the garage, were not provided personal fall arrest systems or other means of fall protection, thereby exposing the employee to fall hazards.  No abatement certification or documentation is required for this item.
