29 CFR 1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   On or about July 23, 2021, at job site 324 S. Poplar Ave., Elmhurst, IL 60126, Employees engaged in construction activities on floor level were not provided a hard hat. Employees at levels of height were engaged in removing siding and throwing it to ground , this exposed employees  at ground level to head injuries from falling objects.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure.   On or about July 23, 2021 at jobsite located in 324 S. Poplar Ave., Elmhurst, IL 60126, employees were exposed to fall hazards of approximately 12 feet while on a pump-jack removing siding from the residence. Fall protection was not being utilized while engaged in residential construction activities.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards:    On or about July 23, 2021, at jobsite located in 324 S. Poplar Ave., Elmhurst, IL 60126, employees performing siding removal at a  residential property were exposed to fall hazards in approximately 12 feet, with no fall protection when the employer failed to ensure they were adequately trained to recognize and minimize the hazards associated with fall.
