29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) U.S. Department of Treasury - United States Mint at Denver, 320 W Colfax Ave., Denver, CO 80204: On and before July 22, 2021, machine specific procedures were not developed and utilized to the control hazardous energy when employees were engaged in servicing and maintenance of the NM02 vertical press. Workers inspected the machine to determine why coins were not feeding and dispensing correctly to two separate locations. The employer did not have a procedure to render the machine safe before inspecting. An employee's right hand was caught in press feed, severely lacerating three fingers.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a) U.S. Department of Treasury - United States Mint at Denver, 320 W Colfax Ave., Denver, CO 80204: On and before July 22, 2021, authorized employee(s) did not receive effective training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy, nor in the methods and means necessary for energy isolation of machinery in the numismatics area. Employees did not recognize that the NM 02 vertical press was required to be locked out and/or tagged out to be rendered safe prior to servicing and maintenance activities. An employee's right hand was caught in the press feed, severely lacerating three fingers.

29 CFR 1910.147(f)(1): Testing or positioning of machines, equipment or components thereof. In situations in which lockout or tagout devices must be temporarily removed from the energy isolating device and the machine or equipment energized to test or position the machine, equipment or component thereof, the following sequence of actions was not followed: (i) Clear the machine or equipment of tools and materials in accordance with paragraph (e)(1) of this section; (ii) Remove employees from the machine or equipment area in accordance with paragraph (e)(2) of this section; (iii) Remove the lockout or tagout devices as specified in paragraph (e)(3) of this section; (iv) Energize and proceed with testing or positioning; (v) Deenergize all systems and reapply energy control measures in accordance with paragraph (d) of this section to continue the servicing and/or maintenance. a) U.S. Department of Treasury - United States Mint at Denver, 320 W Colfax Ave., Denver, CO 80204: On and before July 22, 2021, when servicing and maintenance of the NM02 vertical press required the machine to be energized to inspect the position of the feed and coin discharge, workers did not follow steps to remove all workers from the equipment prior to running the energized machine. An employee's right hand was caught in the press feed, severely lacerating three fingers.
