29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide:   a)  On July  23, 2021, an employee was exposed to the hazard of falling, approximately 20 feet to the ground, while performing masonry tuckpointing  work from a scaffold platform that was not at least 18 inches wide.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.502(d)(6)(i): Snap hooks were engaged directly to webbing, rope or wire rope when not designed for such connections:   a) On July 23, 2021, an employee performing masonry tuckpointing work was exposed to fall hazards when using a personal fall arrest system that had its snap hook engaged back onto the life line.
