29 CFR  1926.1053(b)(1):29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  a)	At the Hathaway's Corners job site , on or about 07/23/2021: Employees were using a portable ladder that was not extended 3 feet above the edge of the roof to access that same roof.

29 CFR  1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system:   a)	At the Hathaway's Corners job site , on or about 07/23/2021: employees were exposed to fall hazards while cutting and installing sheathing at a height of 10' and above without fall protection of any kind.  b) At the Hathaway's Corners job site, on or about 07/23/2021: employees were exposed to fall hazards while erecting trusses at height of 10' and above without fall protection of any kind.
