OSH ACT of 1970 Section (5)(a)(1): The employer failed to furnish to each of his employees employment and a place of employment which was free from recognized hazards that were causing or are likely to cause death or serious physical harm, in that employees were exposed to struck-by hazards: Consolidated Drilling Services LLC, 875 North & 1250 East , Carmi, Illinois 62821: On July 26, 2021, the employer did not ensure that employees were protected from struck-by hazards while using a BJ Type A elevator to remove collar pipes from the well site. A crew of four people was working to remove an approximately 1800 pound, 30 foot long, 6 inch diameter pipe from the well and lay it horizontally in the pipe tub. The crew used a single latch collar clamp with a weak spring to connect the pipe to the elevator. During the process, the clamp loosened, allowing the pipe to fall to the ground, striking an employee in the head.
