29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  A)	American Tool & Die, Inc.  2024 Champlain Street, Toledo, Ohio 43611:  On or about July 20, 2021, the employer did not ensure an energy control program consisting of machine specific control procedures, employee training and periodic inspections were developed and performed.  Employees performing maintenance duties to include die changes at or near the point of operation on the  Bliss 60 ton part revolution  mechanical power press without the benefit of energy isolation, hazardous energy recognition training and employer evaluation, were exposed to laceration(s)/amputation(s) in the event of unexpected press cycle.

29 CFR  1910.217(b)(7)(iii): Mechanical power press(es) using part revolution clutches were not provided with a means of selecting off, inch, single stroke, and continuous (when the continuous function was furnished) with the clutch/brake control to select:  A. American Tool & Die, Inc.  2024 Champlain Street, Toledo, Ohio 43611:  On or about July 20, 2021, the employer did not ensure Bliss 60-ton part revolution mechanical power press cycle selection switch keys were under the control of a supervisor. During production, an operating employee with unabated access to inch mode selection was exposed to amputation hazards while resetting machine to top stroke.

(A)  American Tool & Die, Inc.  2024 Champlain Street, Toledo, Ohio 43611:  On or about July 27, 2021, the employer did not  establish and follow as required an inspection program for a Bliss 60-ton part revolution mechanical power,  Model C-6, S/N  H69967. Employees operating the press were not benefited by a systematic program of periodic and regular inspection of press production systems to ensure that all their parts, auxiliary equipment, and safeguarding are in safe operating condition and adjustment.
