29 CFR  1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.   On or about 07-23-2021 around 2:15 PM, located at Colonna's Shipyard, Inc., 400 East Indian River Road. Norfolk, VA 23523 on Caisson #4 West Yard, an employee's left thumb was caught between a shifted load-bearing component of a scaffold system during disassembly causing a partial traumatic metacarpophalangeal amputation.  The employer did not ensure the employee was adequately trained to recognize the maximum intended load, the load-carrying capacities, and the caught-between and pinch-point hazards of the scaffolds used.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
