29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy: (a) Momentum Recycling, LLC at 11331 Dillon Road in Broomfield, CO: On or before July 27, 2021, the employer did not develop or document energy control procedures that clearly and specifically outlined the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy. Maintenance and operations employees regularly performed servicing and maintenance on equipment that required the use of lockout/tagout. This condition exposed employees to hazardous energy.

29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Momentum Recycling, LLC at 11331 Dillon Road in Broomfield, CO: On or before July 27, 2021, the employer did not ensure that one or more methods of machine guarding was provided throughout the plant on conveyor belts, including conveyor rollers, head pulleys, and tail pulleys, to protect employees working nearby from ingoing nip points. Equipment with inadequate guarding included but was not limited to: - Rollers on conveyors 107, 76, 14, 15, 16, 34, 67, 56, and 61. - Conveyor 67 - unguarded wing tail pulley. - Conveyor 56 - unguarded wing tail pulley. - Conveyor 124 - unguarded tail pulley. - Conveyor 154 - unguarded head pulley. This condition exposed employees to crushing and amputation hazards. (b) Momentum Recycling, LLC at 11331 Dillon Road in Broomfield, CO: On or before July 27, 2021, the employer did not ensure that one or more methods of machine guarding was provided on the rotary airlock to protect employees working nearby from ingoing nip points and rotating parts. This condition exposed employees to crushing and amputation hazards.
