29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. (a) North Bound Bridge, North End: On July 27, 2021 employees were accessing the north in of the bridge via a small concrete walk way that had a jersey barrier on one side and on the other was an unprotected edge that was approximately 6.5 feet above the ground below. Employees were not trained on the fall hazard associated with traveling through this area.

29 CFR 1926.651(b)(2): The Employer did not ensure that maximum allowable slopes and allowable configurations for sloping and benching systems were in accordance with the conditions and requirements set forth in appendices A and B to this subpart. (a) Northbound Bridge, North End: On July 27, 2021, an employee was working in an excavation that was improperly benched starting at 5 feet in depth.
