1910.132(d)(1) The employer did not asses the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment: (a) On or about July 21, 2021, employees working on and around the Starview Packaging Machine (Model No. FAS4-1012 and Serial No. 3169) were not protected from a crushing hazards. The blister grabber was not properly guarded to protect employees from a pinch point.
