29 CFR 1926.303(b)(1): Grinding machines were not equipped with safety guards in conformance with the requirements of ANSI B7.1-1970, Safety Code for the Use, Care, and Protection of abrasive wheels, and paragraph (d) of this section. a) 1300 Stedman St. - On or about July 21, 2021, employees operated a Milwaukee M18 Fuel cordless grinder (Serial Number: K02AD206000000) with the guard removed, exposing employees to caught-in and struck-by hazards.

29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high. a) 1300 Stedman St. - On or about July 21, 2021, employees performed ductwork installation in the fan room near a window opening with an unprotected edge of greater than six feet. A mid-rail, screen, mesh, or equivalent intermediate structure members were not installed between the top edge of the guardrail system with the wall at six (6) inches in high. This condition exposed employees to fall hazards of greater than six (6) feet to the grade below.
