29 CFR  1926.453(b)(2)(v):  A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift.  On or about 7/27/2021, employees from Panda Roofing LLC were working on a canopy at 3235 Mid Rivers Mall Drive in St. Peters, MO. The crew lead and an employee were attaching materials to the roof. The employees were using a JLG Lift, Model 460SJ - Serial # 0300273620, to access the roof. The crew lead was operating the lift and using it while installing the materials on the edge of the roof. Neither employee was wearing a body belt with lanyard attached to the boom or basket while using the aerial lift.
