29 CFR  1910.119(f)(4):The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping:  On or about July 27, 2021 workers at the LyondellBasell chemical manufacturing facility in La Porte, Texas were exposed to process materials, toxic release, and flammable hazards from the Acetic Acid Unit when the host employer did not implement safe work practices to control hazards during modification of HV-117 for LOTO and repair of the Methanol supply line to the Acetic Acid Reactor.

29 CFR  1910.147(c)(4)(ii)(B): Energy control procedure was not developed and implemented  to clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.  On or about July 27, 2021 in the LyondellBasell Chemical Manufacturing Complex in La Porte, TX workers were exposed to process materials, toxic release, and flammable hazards when energy control procedures were not developed and implemented in the Acetic Acid Unit to block and isolate HV-117 from hazardous energy sources while it was being worked on.

29 CFR  1910.119(h)(2)(ii):The employer did not inform contract employers of the known potential fire, explosion, or toxic release hazards related to the contractor's work and the process:  On or about July 27, 2021 workers at the LyondellBasell (LYB) chemical manufacturing facility in La Porte, Texas were exposed to process materials, toxic release, and flammable hazards from the Acetic Acid Unit when LYB did not inform its contractor (Turn2) that the Acetic Acid Reactor would not be de-inventoried and/or isolated from HV-117 while work was occurring on HV-117 and did not warn that the equipment arrangement and operating state creates an increased / elevated potential for unexpected energy release at HV-117 that could result in fire, explosion, and/or toxic release if the pressure boundary of HV-117 is compromised during their work.

29 CFR  1910.119(j)(2):The employer did not establish or implement written procedures to maintain the on-going integrity of process equipment.  On or about July 27, 2021 at the Lyondell Basell Chemical Manufacturing facility in La Porte, Texas workers were exposed to process materials, toxic release, and flammable hazards from the Acetic Acid Unit when the employer did not establish and implement written procedures to direct the workers in modifying  HV-117, which was installed in process but not isolated, so that it could be used as an isolation point in the LOTO of the methanol supply piping to the Acetic Acid Reactor.

29 CFR  1910.119(l)(1):The employer did not implement procedures to manage changes to process chemicals, technology, equipment, and procedures, and changes to facilities that affect a covered process.  On or about July 27, 2021 at the LyondellBasell Chemical Manufacturing Complex in La Porte, Texas, workers were exposed to process materials, toxic release, and flammable hazards when the employer did not implement procedures to manage change when it made modification to HV-117 in the Acetic Acid Unit.  (a) Change to usual operating practice of requiring positive isolation or double block and bleed when employer elected to use of HV-117 as single isolation for the methanol line without documented approval of Engineering, Operations, Safety, and Management. (b) Change to normal practice when implemented modification of HV-117 without input/approval of Engineering, Operations, Safety, and Management (c) Change to normal operating practice when employer did not de-energize process equipment prior to allowing it to be serviced
