29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i): (a) WET DRAW AREA, ABOVE MACHINE HBZ01 - Employees who maintained the equipment on the electrical cabinet mezzanine were exposed to an 11 foot fall hazard through the approximate 18 inch gap between the end of the guardrail and the railing of the ladder stand that was used to access the mezzanine, on or about August 13, 2021. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

(d) DRY DRAW AREA, BACK ROOM - The employer utilized an extension cord to provide electrical power to the Hisense AC unit and a temperature sensor that was being used to cool the electrical cabinet for the dry draw wire equipment that was installed on the North wall, on or about July 28, 2021. (e) WET DRAW AREA, HBZ02, HBZ03, HBZ04, HBZ05 WIRE DRAWING MACHINES - The employer utilized two extension cords in series to provide electrical power to the finish wire lubrication pump installed on each of the HBZ wire drawing machines, on or about July 28, 2021. (f) WET DRAW AREA, HBZ02, HBZ03, HBZ04, HBZ05 WIRE DRAWING MACHINES - The employer utilized a three outlet extension cord to provide electrical power to the lubrication circulation pump, the finish block coolant circulation pump, and the coolant feed water pump on each of the HBZ wire drawing machines, on or about July 28, 2021.
