29 CFR  1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5).  Warehouse - the employer failed to ensure that each forklift operator received formal instruction, practical training, and evaluation of the operator's performance in the workplace prior to starting regular material handling tasks. Employees were exposed to hazards associated with unsafe forklift operation and load handling.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
