On or about July 30, 2021, on the south side of the main fabrication bay; employees utilized a HEM Saw (S.N. # 1189314) to cut metal during machine operations that were not equipped with a guard.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to an outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). On or about July 30, 2021, employees engaged in machining operations were exposed to electrical shock hazards due to the employer failing to visually inspect the flexible cord used to power hand tools.
