29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:   Commercial Building: On or about July 28, 2021, and at times prior thereto, workers were exposed to a fall hazard, while operating a Genie S-85 Telescopic Boom Lift without being trained on its safe operation and fall hazard recognition.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  Commercial Building:  On or about July 28, 2021, and at times prior thereto, employees without fall protection were exposed to a fall hazard greater than 6 feet to the unpaved ground below while metal decking on the roof.  Gomez Metal, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1534913, citation number 1, item number 1 and was affirmed as a final order on August 4, 2021 with respect to a workplace located at 17627 Overlook Loop, San Antonio, TX 78247.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards that would enable each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards:  On or about  July 28, 2021, employees without fall protection were exposed to a fall hazard greater than 6 feet to the ground below, while metal decking without being trained in recognizing and addressing fall hazards.  Gomez Metal, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.503(a)(1), which was contained in OSHA inspection number 153491, citation number 1, item number 2 and was affirmed as a final order on August 4, 2021, with respect to a workplace located at 17627 Overlook Loop, San Antonio, TX 78247.
